Intra-arterial therapy for large vessel stroke

This study (MR CLEAN) has the potential to change stroke therapy considerably. Patients with proven large vessel occlusion were nearly all treated with tPA (very early – mean time of 90 minutes after symptom onset) then randomised to receive clot retrieval (nearly all within 6 hours of onset) and mostly with retrievable stents or not. The rate of functional independence at 90 days in the treatment group was 32.6% vs. 19.1 in the control group – which is pretty impressive for a group of patients who usually do not usually do very well and respond poorly to tPA.
