Hint: you'll need to correlate the electro- and mechanical activity of the heart.
This is an atrial tachycardia (135) with a 2:1 AV block and some non-specific AT-TW changes.
The throbbing? Easy. Every other atrial contraction occurs while the tricuspid valve is closed (canon wave).
